The
Capital 401(k) Investment Plan Administrator beginningJanuary 1, 2003 is
the Principal
Financial Group. You can access your account through the
above Principal Web Site. You can also access your account through the
Voice Response System at 1-800-547-7754. You will
need your Social Security
Number, the product # (450273) and your password
(PIN) to access your
account information. You must establish your PIN (directions can be accessed
through the Capital Web Site,
in the Employee Section under Employee Benefit
Forms, 401(k) Forms - English
or Spanish).
* Changes to your Plan Investment Fund choices can be made at any time
through the Web Site or Voice Response System (phone).
* Changes to your payroll deferral percentage, after you have completed
the
initial enrollment form, must be made on the 401(k) % Change Form (access
this form through the Capital Web Site in the Employee Section under Employee
Benefit Forms, 401(k) Forms - English or Spanish)
and forward the completed
form to the Corporate Human Resource Department
in Phoenix, Arizona. The
change will be effective the first payday of the following quarter.
* Changes to your Beneficiary can be made at any time and must be made
on
the Beneficiary Form (access this form through the Capital Web Site in the
Employee Section under Employee Benefit Forms, 401(k)
Forms - English or Spanish).
Complete the form and forward it to the Corporate Human Resource Department
in Phoenix, Arizona.
The Matching
Funds for 2003 will be 45% of the first 5% of your annual
payroll deferral.
* Investment
Manager remains as Miller/Russell & Associates. They can be
contacted at 1-800-222-1232 if you have questions regarding your Investment
Fund Choices.
* The Custodian of your Plan Funds is now Delaware Charter Guarantee &
Trust
Company (Trustar).
* Capital Human Resource Department in Phoenix can be contacted if you have
questions regarding your Plan features - 1-800-966-0709

* Chiropractic claims must be pre-certified after the initial evaluation by
calling Chiropractic Cost Containment System (CCCS) at 1-800-680-9997.

* Use a doctor, hospital, lab, clinic that is in the network to obtain maximum
claims payment benefits.

* Carry the Prescription formulary pocket guide with your Medical ID card.
* Tell your doctor that you have a prescription formulary.
* Mail order prescription service is available and can save you additional
money.
* You can order 3 month's supply of medication for 2 co-pays.
* Your Division can provide the form and information.
* To access the Wausau Benefits web site and prescription formulary click
the Wausau hot link above, then click on the products bar on the left
and then Wausau Pharmacy Benefits bar.
DENTAL
* $1,200 maximum per person/per year benefits (no Orthodontic
coverage).
* You may use any dentist you wish.
* Using Dental Benefit Providers (WDMD) Network will provide a discount
to you.
*You can contact Dental Benefit Providers (WDMD) 1-800-826-9781, extension
2054.
* Mail ALL
dental claims to Wausau Benefits, P.O. Box 8014, Wausau, WI 54402-8014.
*To access the Dental Benefit Provider Network, click on the Wausau button
above and then click on "Find a Provider" bar on the left and
then Dental Networks.
* The EyeMed vision plan is a voluntary, elected benefit
that has a 3-tier premium cost.
* If you have elected this benefit, you have received an EyeMed ID card
that was mailed to your
home address.
* To obtain maximum benefits use a provider in the EyeMed Plan.
* You can access the
EyeMed Web Site and Provider Locator through the hot link above.
* Non-participants, who would like more information about EyeMed or want
