medical / dental and prescription plans are self-funded through
Capital.
• The plans are administered by United Medical Resources (UMR).
• Online services from UMR include access to participant benefit information
including medical/dental, pharmacy and flexible spending account.
• Access the Medical Provider network at Unitedhealthcare Options PPO at
www.umr.com or call 866-868-7406.
• Access the Dental Provider network at Dental Benefit Providers / DBP
at www.umr.com or call 866-868-7406 or continue to use current provider.
• Access the Pharmacy plan at www.umr.com through Innoviant at 877-559-2955.
• 24/7 NurseLine an advance health information service is available at
800-808-4424 (must have participant ID number for access).

BENEFITS

EyeMed vision plan is a voluntary benefit. The plan is structured with
a 3-tier premium cost.
• EyeMed ID cards are mailed to participants home address.
• Use a provider in the EyeMed High / Select Option Plan for maximum benefit.
